Owner Authorization
I certify that I am at least 18 years of age and am the owner of, or authorized representative for, the pet(s) receiving care. I have the authority to make medical and financial decisions regarding their veterinary care.
Authorization for Veterinary Care
I authorize Animal Care Innovations, Ltd. d/b/a Tandem ("Tandem," "we," "us," or "our"), its veterinarians, veterinary technicians, and other authorized team members to provide routine veterinary services for my pet(s), including examinations, preventive care, diagnostic testing, medical treatment, hospitalization, and other services that I approve of.
Recommendations for additional diagnostics, treatments, procedures, medications, or hospitalization may require your approval before they are performed, except as otherwise authorized or when immediate action is necessary to protect your pet’s health or welfare.
Risks of Veterinary Care
I understand that all examinations, diagnostic tests, treatments, medications, and procedures involve inherent risks, and that unexpected complications or outcomes may occur despite appropriate veterinary care.
I understand that Tandem cannot guarantee any particular diagnosis, treatment outcome, or recovery.
Certain procedures or treatments may require additional authorization or consent before they are performed.
Financial Responsibility
I agree to accept financial responsibility for veterinary services that I authorize.
Financial terms, including estimates, deposits, payment methods, and payment timing, may be documented separately from this General Treatment Authorization.
Communication
I authorize Tandem to communicate with me regarding my pet’s care using the contact information associated with my account, including telephone, text message, email, in-app chat or other electronic communication methods that I have provided.
If I designate an alternate contact, I authorize Tandem to communicate with that individual regarding my pet’s care. If I expressly authorize that individual to make medical decisions on my behalf, Tandem may rely on those decisions until I revoke that authorization.
Telemedicine
I understand that Tandem may offer veterinary services through telemedicine when appropriate and permitted by applicable law.
I acknowledge that telemedicine has inherent limitations, including the inability to perform a hands-on physical examination or certain diagnostic tests. I understand that Tandem may determine that an in-person examination is necessary before making a diagnosis, prescribing treatment, or continuing care.
I understand that telemedicine is not appropriate for all medical conditions and should not be relied upon for emergencies. If my pet is experiencing a medical emergency, I will seek immediate in-person veterinary care.
Relationship to Tandem
This General Treatment Authorization is an agreement with Animal Care Innovations, Ltd. d/b/a Tandem ("Tandem," "we," "us," or "our"). This document is binding on Animal Care Innovations, Ltd. d/b/a Tandem, its successors, and assigns.
